Immunizations
The Ohio Department of Health regulates the rules governing the immunization.

Students must have proof of the following immunizations before starting school. If proof is not received by this date, students will be excluded from school until compliance is met.
Requirements for Preschool
     ~hepatitis B (3 in a series)
     ~1 MMR
     ~4 DTP/DTaP/DT/TD
     ~3 Polio

Requirements for Kindergarten through 11 th grade
     ~hepatitis B (3 in a series)
     ~2 MMR
     ~5 DTP/DTaP/DT/Td if the fourth dose was administered prior to the fourth birthday
     ~4 Polio if the third dose was administered prior to the fourth birthday
     ~All KG-2 students - Varicella (chickenpox) House Bill 463 requires kindergarten students "during or after the school year beginning in 2006” to provide evidence of having received immunization against chicken pox.  If a student has had natural chicken pox, written evidence from parent/guardian or physician may be accepted.

Vision (Distance Visual Acuity Test)

Hearing Testing Requirements Screen with a pure tone audiometer at 20 dBHL
      *1000 Hz – Right & Left Ear
      *2000 Hz – Right & Left Ear
      *4000 Hz – Right & Left Ear

Scoliosis (not mandated)

  • 6th grade students

Height, Weight, Body Mass Index (BMI) (not mandated)
      not yet in practice-as required by the State of Ohio Department of Health

Head Lice   
The Southern Local School District has a “nit-free” policy.

  • WHEN CHECKED (by the Meigs County Health Department)
     *Beginning of school
     *After Christmas vacation
     *After Easter break
  • WHEN FOUND
     *Temporarily exclude from school until after the first treatment or nits removed
     *Treat with RID, Nix, De-Bug, or a prescription from the family physician
     *Nits must be removed from hair
     *Vacuum carpets, furniture and house daily
     *Wash clothes, hats, scarves, and bedding.

Communicable Diseases

  • Pink Eye - Exclusion from school for 24 hours after treatment with antibiotic
  • Impetigo - Exclusion from school for 24 hours after treatment has begun and there is no discharge
  • Chicken Pox - Exclusion from school until all lesions are scabbed over and dry (5-7 days)
  • Ringworm - Exclusion from school until treatment has begun, and lesions must be covered on return to school
  • Scabies - Exclusion from school for 24 hours following the initial treatment with appropriate scabicide
